User Name: Your username will be your last name followed by the last 4 digits of your SSN. For example, John O'Hara or John Setz-Kelly, SSN 123-45-6789 will have a username of OHARA6789 or SETZKELLY6789 (name in all CAPS).
NOTE: If your name contains an apostrophe or hyphen, do not enter it.

Password: Your initial temporary password will be the 4 digit year of your birth followed by the first two letters of your first name. For example, if John Doe was born on January 1, 1960, his password is 1960JO (name portion in all CAPS).
