Whether you are a participant of the Ohio Conference of Teamsters & Industry Health & Welfare Fund, or a participant of the Ohio Contractors Association Health & Welfare Fund, rest assured that we've got you covered! Most of our Plans feature generous and comprehensive benefit packages that generally include Medical, Prescription Drug, Dental, Vision, Short Term Disability (Loss of Time), Life and Accidental Death and Dismemberment (AD&D) benefits. Protecting your personal health information is of utmost importance to us. You Get Married You Get a Divorce You Have Children You Lose Coverage Your Child Turns 26 You Stop Working in Covered Employment Find a Network Provider or Pharmacy Medical Mutual Posted November 11, 2020 The Fund uses the Medical Mutual PPO provider network, allowing you to see any provider you choose without needing a referral to see a Specialist or designating a Primary Care Physician before seeking care. Medical Mutual has special arrangements and receives certain discounts from participating providers, which means if you use an in-network provider, you also pay a lower coinsurance percentage. Participants residing outside of Ohio have only the in-network level of benefits. The out-of-network level of benefits does not apply to out-of-state participants. To locate a Network Doctor or Hospital near your home, visit the Medical Mutual Website . CVS Caremark® Posted March 1st, 2022 The Ohio Conference of Teamsters & Industry Health and Welfare Fund has partnered with CVS Caremark to manage your prescription benefits. Register at Caremark.com, where you can see plan information, check drug cost and coverage, find personalized drug savings opportunities and more. For more information and answers to commonly asked questions, visit Caremark.com/HelpCenter or call CVS Caremark customer care at 1-844-449-0353. To have your prescription filled, simply present your CVS Caremark member ID card at any in-network pharmacy nationwide (including those inside Target stores). EmpiRx Posted April 30, 2024 The Teamsters Ohio Contractors Association Health & Welfare Fund uses EmpiRx as its Plan's Prescription Benefit Manager to provide you the Prescription Drug benefits. Should you have any questions about your pharmacy or participating pharmacies in your area, please feel free to contact EmpiRx member services at 1-877-241-7123 or visit www.empirxhelath.com. To have your prescription filled, simply present your drug identification card, and make your co-payment. Delta Dental of Ohio Posted March 1st, 2022 The plan uses the Delta Dental PPO ™ and Delta Dental Premier ® networks. Seeing a dentist in the Delta Dental PPO network will offer you the best savings. You may go to any licensed dentist; however you may pay more out of pocket if you choose a nonparticipating dentist. Find a Delta Dental provider online or by calling customer service at 1-800-524-0149. Make an appointment to see your Delta Dental dentist, and let them know you are a Delta Dental of Ohio member. If your dentist is not currently in a Delta Dental network, you can refer your dentist on our website by entering your dentist’s information in the dentist referral section. National Vision Administrators Posted March 1st, 2022 National Vision Administrators (NVA) has been selected as a partner to provide you and your family vision benefits. You can go to any vision care provider, however, using a NVA network provider will save you money and give you the greatest benefit under the Plan. Finding a Network Provider is easy. Visit www.e-nva.com and select the Find a Provider link. Enter your group number and your search parameters. You don’t even need an ID Card when you see a provider. Just tell your vision care provider that you have benefits through NVA. Contacting NVA's US-based Customer Service team 24/7/365 by calling 1-800-672-7723 for questions about your benefits, claims status or network providers near you. Payer Matrix Posted April 30th, 2024 Specialty Medication Management Program The Fund has partnered with Payer Matrix to offer assistance and relief to high dollar specialty drug costs. Where applicable, alternative funding for select high-cost specialty drugs may be obtained, and your out-of-pocked costs could be reduced or eliminated. If you or your dependents are taking one of the eligible drugs, you must enroll in the Payer Matrix program or you will be responsible for the full cost of the medication. Contact Payer Matrix at 1-877-305-6202 or via email at customerservice@payermatrix.com for details or to enroll. What to do When...? You Get Married If you get married and would like to add your spouse to your plan, you must fill out a new enrollment online and submit a copy of your marriage certificate, within 30 days of the date of your marriage. Your employer may require a higher payroll deduction to add a spouse to your plan, so be sure to check your Collective Bargaining Agreement for more details. What to do When...? You Get Divorced If you get divorced, you must complete a new enrollment form online, changing your election and terminating coverage for your former spouse. You must do this within 60 days of the date of your divorce. The Fund office also requires that you submit a copy of your Divorce Decree. If your former spouse was covered under your health plan, he or she is entitled to COBRA benefits under the Plan. The Fund office will send him or her a COBRA informational packet once we receive notice of the divorce and terminate coverage. What to do When...? You Have a Child If you have a child(ren) and would like to add your child(ren) to your plan, you must enroll them within 30 days of birth and submit a copy of the birth certificate. If you miss the 60-day window to enroll your child(ren), then you must wait until the following Open Enrollment period unless you have another qualifying life event. Your employer may require a higher payroll deduction to add child(ren) to your plan, so be sure to check your Collective Bargaining Agreement for more details. What to do When...? You Lose Other Insurance Coverage If you or an eligible dependent have group coverage through another employer or through a spouse or parent and lose that coverage, you will have 30 days from termination of the other coverage to enroll into this Plan. You must enroll online and provide proof of coverage termination before we can activate benefits under the Plan. What to do When...? Your Child Turns 26 If your children are eligible for coverage under your plan, they are eligible to remain covered until the last day of the month in which they turn age 26. Once they lose coverage under the Plan for attaining the age of 26, they may be eligible to receive COBRA coverage by making self-payments. The Fund office will send a COBRA information packet about the COBRA option to your dependent. If your dependents do not live with you, it is important you notify the Fund office of their address. If as a result of your dependent(s) coming off of your coverage, your election changes from Family to Single +1, or From Single+1 to Single, your employer required payroll deductions toward your coverage may also be reduced. Check your Collective Bargaining Agreement for more details. What to Do When? You Stop Working in Covered Employment If you stop working in covered employment, your health coverage will end on the last day of the month, or on Sunday of the week your employment ends if your eligibility is determined weekly. Please check your Collective Bargaining Agreement or call the Fund office to find out if your coverage is determined weekly or monthly. Once your Employer notifies the Fund of your termination date, the Fund office will send you information on your eligibility to elect COBRA coverage (if applicable).
