OCA Life and Accidental Death and Dismemberment

In the unfortunate event of your death while covered under this Plan, the Fund will pay your designated beneficiary a $25,000 Death Benefit. Proof of death must be filed with the Fund Office in order to receive this benefit. The benefit is payable for death by any cause.
If there is no Designation of Beneficiary Form on file with the Fund Office at the time of your death, or if the beneficiary you have named is no longer living, intentionally caused your death, or is a spouse from whom you are legally divorced, then the benefit shall be paid to your estate, unless otherwise provided in a final divorce decree.
Beneficiary Designation
In order to name a beneficiary, you must submit a Designation of Beneficiary Form. You may change your beneficiary at any time, and as often as you wish, by submitting a new form. Any designation or change of beneficiary will become effective when the completed form is received by the Fund Office; however, no change will be effective if it is received after the date of death.
Extension of Benefit for Total and Permanent Disability
If you become disabled, you will continue to be covered by the Plan for up to 12 months if you furnish proof of your disability to the Plan Office within 30 days of your last day at work, subject to Plan rules. This benefit will end once you have not been actively at work due to your disability for 12 months.
After your benefits end, Death Benefits may still be payable to your beneficiary if:
- You become Totally Disabled (as defined by the Plan) before your benefits end;
- You are less than 50 years old when you become Totally Disabled;
- You continue to be Totally Disabled after your benefits end and until the date of your death; and
- You submit the required proof.
For Death Benefits to be payable, you must furnish proof of Total Disability not later than 12 months after the date you ceased to be actively at work because of Total Disability. This proof must establish that your Total Disability continued for at least nine months from the date you were last actively at work. You must submit proof of continued Total Disability to the Fund Office each year. Coverage can be continued until you reach age 65. However, if you become disabled after age 59, you can continue coverage to age 70 or five years after the date you first became disabled, whichever occurs first.
Death Benefits will end on the date you are no longer disabled, or the date you do not give the Fund Office or the Plan's insurance carrier proof of Total Disability when required.
Total Disability means that because of a sickness or an injury you cannot do your job; and you cannot do any other job for which you are fit by your education, training or experience.
This extension for Total and Permanent Disability are not available if you are eligible for a conversion policy.
The provisions on Death Benefits are subject to any further terms of the group life insurance policy purchased by the Plan, a copy of which is available for your review at the Fund Office.
Accidental Death and Dismemberment Benefits
If you are involved in an accident on or off the job which results in loss of life, loss of limbs or loss of sight through external, violent, and accidental means, an Accidental Death and Dismemberment Benefit becomes payable to you or your beneficiary, in addition to any other benefits payable under this Plan, provided that you did not contribute directly or indirectly to the cause of the loss.
Depending on the extent of the loss, the following benefits are payable:
- Loss of Life - $25,000
- Loss of two limbs, sight of both eyes, or
Loss of one limb and sight of one eye - $25,000
- Loss of one limb or sight of one eye - $12,500
Loss of limb means severed at or above the wrist or ankle joint. Loss of sight means the total and irrecoverable loss of sight. For benefits to be payable, any loss must occur within 90 days from the date the injury first happened.
If more than one of the losses listed above is suffered as the result of any one accident, only the maximum amount of benefit for Accidental Death and Dismemberment will be payable.
Benefit Exclusions and Limitations
Benefits shall not be payable for any loss which is due to the exclusions set forth in your life insurance policy from time to time.
The provisions on Accidental Death and Dismemberment Benefits are subject to any further terms of the group life insurance policy purchased by the Plan, a copy of which is available for your review at the Fund Office.
