Life Insurance
The Plan's Life Insurance benefit is fully insured by MetLife. In the unfortunate event of
your or your dependent's death (if your plan allows for dependent coverage) while covered
under this Plan, the Fund will pay the designated beneficiary a Death Benefit. The amount of
the Death Benefit varies by Plan. Please reference the Summary Plan Description booklet in
your enrollment packet or on the Documents page of our website.
Beneficiary
Designation
You can designate a life insurance beneficiary on your enrollment form when you become eligible
for benefits. To change your designated beneficiary at any time, simply submit a Designation
of Beneficiary Form online. Any designation or change of beneficiary will become
effective when the completed form is received by the Fund office; however, no change will be
effective if it is received after the date of death.
Payment of
Benefits
Upon your death, your designated beneficiary must contact the Fund office to initiate the claim
process. Proof of death in addition to the appropriate claim form must be provided to the Fund
office timely. The Fund office will work with MetLife to facilitate the claim submission process
and ensure proper documentation is in order.
If there is no Designation of Beneficiary form on file with the Fund office at the time of your
death, or if the beneficiary you have named is no longer living or, intentionally caused your
death, then the benefit shall be paid according to a schedule of assignment, unless otherwise
directed by a legal document.
Accidental
Death and Dismemberment Benefits
The Plan's Accidental Death and Dismemberment benefit is also fully insured by MetLife. If you
are involved in an accident on or off the job which results in loss of life, loss of limbs or
loss of sight through external, violent, and accidental means, an Accidental Death and
Dismemberment Benefit becomes payable to you or your beneficiary, in addition to any other
benefits payable under this Plan, provided that you did not contribute directly or indirectly to
the cause of the loss.
Additional
Benefits
- Seatbelt Benefit - If the covered individual dies as a result of injuries
sustained from an accident while driving or riding as a passenger in a passenger car and
wearing a seat belt which was properly fastened at the time of the accident, an
additional 10% of the full life insurance benefit is paid to the beneficiary.
- Air Bag Benefit - If the covered individual dies as a result of injuries
sustained from an accident while driving or riding as a passenger in a passenger car
equipped with an Air Bag(s), was riding in a seat protected by an air bag, and was
wearing a seat belt which was properly fastened at the time of the accident, an
additional 5% of the full life insurance benefit is paid to the beneficiary.
- Common Carrier Benefit - If the covered individual dies as a result of an
accidental injury resulting in death while traveling in a Common Carrier, an additional
benefit will be paid. A common carrier means a government regulated entity that is in
the business of transporting fare paying passengers.
- Repatriation Expense Benefit - If the covered individual dies as a result of an
accidental injury at least 100 miles from their place of residence, an additional
benefit equal to the charges incurred for the preparation and transportation of the
deceased's body to the city of the deceased's principal residence; not to exceed $5,000.
Benefit
Exclusions and Limitations
Benefits shall not be payable for any loss which is due to the exclusions set forth in your life
insurance policy from time to time.
The provisions on Accidental Death and Dismemberment Benefits are subject to any further terms of
the group life insurance policy purchased by the Plan, a copy of which is available for your
review at the Fund Office.
