Summary Annual Report (SAR)

plan.
Summary Plan Description (SPD)
The SPD is your primary guide to the plan. It explains how the plan works, what benefits are
offered, who is eligible, and how to use your benefits. It also outlines your rights and
responsibilities under the plan.
Summary of Material Modifications (SMM)
This document highlights any important changes made to the plan. If benefits, rules, or
procedures are updated, the SMM explains what has changed and how it may affect you.
Summary of Benefits and Coverage (SBC)
This document is a simple standardized summary of the health benefits services that are
covered and what you will pay in deductibles, coinsurance and co-pays.
