Kansas

The Plan offers a Preferred Provider Organization (PPO) through BlueCross BlueShield of Kansas. Contact BCBS at 1-800-432-3990 or at www.bcbsks.com to find a participating provider.

Kansas City Metro/Missouri

The Plan offers a Preferred Provider Organization (PPO) through Preferred Health Professionals (PHP). You can contact PHP at 1-800-544-3014 or at www.phpkc.com to find a participating provider.

Prescription

The Plan offers prescription drug benefits through SavRx. The SavRx pharmacy network includes most major chain pharmacy locations, where you can fill your prescriptions. Please remember that any prescriptions filled at Wal-Mart or Sam’s Club are not covered under the Plan. You may contact SavRx at (800) 228-3108.

Utilization Management (UM)

The Plan offers a Utilization Management (UM) Program through HealthSmart. If you are going to be in-patient the Provider must call HealthSmart to pre-authorize the care. If they do not make this call, the benefits payable by the Plan may be reduced. In case of an emergency admission, you or a family member must call HealthSmart by the next business day after the admission. Call HealthSmart at (877) 202-6379.

Dental

You may use any dental provider for services, however the Plan will pay more if you use a PPO dentist. As a participant of the Plan, you have access to the services and benefits of Delta Dental, which offers discounted rates for your dental care needs. You may contact Delta Dental at (800) 234-3375 or at www.deltadentalks.com

Vision

You may use any vision provider for services, however the Plan will pay more if you use a VSP provider. As a participant of the Plan, you have access to the services and benefits of VSP, which offers discounted retail rates for your vision care needs when you go to any provider participating in the network. Simply tell your eye doctor that you are a VSP participant and you will receive instant discounts on frames, lenses, contacts and exams. To verify that your vision care provider participates in VSP, ask them, or call VSP at (800) 877-7195.

Short Term Disability

The Plan provides a Short Term Disability Benefit. If you are totally disabled you may be eligible to receive this benefit. You are considered totally disabled if you are completely unable to perform the regular duties of your job because of a non-occupational injury or illness and are under the direct and continuing care of a physician. If you are eligible for this benefit, you should call the Fund Office as soon as possible at (785) 267-0140.

Death Benefit

In the event of your death, the Plan may provide a death benefit to your beneficiary. The amount of your death benefit is $10,000.00 for the member and $5,000.00 for spouse or dependents. The death benefit will be paid to your beneficiary as designated by you on the form authorized by the Fund Office, provided the Fund Office receives your completed form prior to your death. A beneficiary designation form is available at the Fund office, you can request a form from the Fund Office at (785) 267-0140.

Schedule of Benefits

Quality Preferred Provider Organizations have agreed to discount services provided to Plan Participants by participating preferred providers, reducing benefit costs for both the Plan and the participant. The higher coinsurance for care provided by non-Preferred Providers does not begin to cover the increased costs to the Plan when using a non-Preferred Provider.
