Pension Verification Form

For members currently receiving monthly pension payments:

You are required yearly to fill out a Verification of Pension Information form. This form will be sent out to you via mail each fall (September through November). We are providing you with an electronic version (click the link below) as an alternative for not receiving the form in the mail.

After reviewing this form, please answer all requested information, sign in front of Notary, and return to this office.
